Hospital Bridge Program
Empowering Transitions Through Peer Support
The Hospital Bridge Program at New Hope C.O.R.P.S., Inc. is designed to assist individuals with substance use disorders (SUD) and co-occurring mental health conditions as they transition from inpatient hospital care back into the community. This program is built on a foundation of shared lived experience, empathy, and the belief that recovery is possible for everyone.
We currently partner with several local emergency departments and behavioral health units, including:
Baptist Homestead Hospital, Larkin Community Hospitals, South Miami Hospital, Jackson South, and Jackson Behavioral Health Hospital.
Peer-Led Engagement & Navigation
At the heart of the program are our Peer Support Specialists—individuals in sustained recovery who draw from their personal journeys to inspire hope and provide meaningful support. Their role includes:
- Building trust and rapport with patients during hospitalization
- Offering encouragement and practical guidance for early recovery
- Collaborating with hospital staff on individualized discharge and recovery plans
- Accompanying individuals to follow-up appointments, treatment admissions, and other essential services in the community
Continuity of Care After Discharge
To promote long-term stability and reduce the risk of relapse or readmission, the program offers comprehensive post-discharge support, made possible through strong partnerships with hospitals, behavioral health providers, and Thriving Mind South Florida. This includes:
- Seamless referrals to detox, residential, outpatient, and medication-assisted treatment programs
- Connections to housing, food assistance, and transportation resources
- Ongoing recovery coaching and personalized follow-up to ensure continued engagement in care
